Exosomes for hair and scalp: an honest status report

Exosome scalp treatments are among the fastest-growing services in aesthetic clinics. The evidence is real but early. Here is where it stands.

The idea

Hair follicles cycle between growing, regressing and resting phases. In thinning scalp, more follicles sit in the resting phase and the ones that grow produce finer hair. Laboratory work has shown that vesicles from dermal papilla cells and from mesenchymal stromal cells carry signals that push follicles from resting into growth, and in mouse models this translates into visibly faster regrowth.

The human data

Human evidence consists mainly of small case series, typically 20 to 40 patients, receiving topical MSC-derived exosomes after scalp microneedling every few weeks for three to six months. Most report increases in hair count and shaft thickness on phototrichogram, and high patient satisfaction. The limitation is that few include a control group receiving microneedling alone, and microneedling by itself is known to improve hair density. Until controlled trials report, the size of the exosome contribution is uncertain.

How clinics use it

  1. Scalp microneedling at a depth appropriate for the scalp, usually 1.0 to 1.5 mm.
  2. Topical application of the exosome preparation across the treated area immediately afterward.
  3. Sessions every three to four weeks for four to six sessions, then maintenance every few months.
  4. Standardised photographs and, where available, phototrichogram at baseline and after the course.

Some clinics inject exosomes into the scalp. As with skin, that is an unapproved use in the United States and we do not supply products for it.

Setting expectations

Tell patients that the treatment is an adjunct, not a cure; that results take months and are modest; that maintenance is needed; and that medical options such as minoxidil and finasteride, prescribed by a physician, have far stronger evidence and can be used alongside. Patients who hear this and choose to proceed are the ones who will be satisfied.

What we are watching
