Where exosomes are used

Five areas, from products on shelves today to research that is years from any clinic. Each one comes with a plain statement of how much evidence there is.

How to read the evidence notes

"Established" means results have been repeated across independent groups and, where relevant, in people. "Early" means most of the work is in cells or animals, or in small human studies without control groups. "Speculative" means the idea is sound but data in people is thin or absent. Marketing rarely uses these distinctions; we do.

Topical skincare

Evidence: early

This is the largest consumer category. Serums, ampoules and post-procedure products contain exosomes from cultured human cells, animal cells, or plants. They are sold as cosmetics, which means they may claim to moisturise, improve appearance or support skin, but not to treat a condition.

What is reasonably supported: exosomes from mesenchymal cells carry growth factors and microRNAs that, in cell culture, encourage fibroblasts to produce collagen and reduce inflammatory signalling. Several small clinical studies have reported faster recovery of redness and barrier function after laser or microneedling when an exosome preparation was applied afterward.

What is not established: whether intact exosomes penetrate unbroken skin in meaningful amounts. Most positive human data comes from application onto skin that has just been treated with a laser or needling device, where the barrier is temporarily open. For daily use on intact skin, the effect of the exosomes themselves, as opposed to the rest of the formula, has not been separated out well.

Hair and scalp

Evidence: early

Exosome scalp treatments are offered in clinics, usually applied topically after microneedling or occasionally by injection. Interest comes from laboratory work showing that vesicles from dermal papilla cells and MSCs can push hair follicles from the resting phase into the growth phase in animals.

Human data consists of small case series without control groups. Some report increases in hair density over several months. Because these studies rarely compare against the microneedling alone, the exosome contribution is uncertain. Injected scalp treatments are also subject to the regulatory concerns described below.

Wound healing and tissue repair

Evidence: early to established in animals; early in people

This is where much of the serious research sits. In animal models, MSC-derived exosomes have repeatedly accelerated closure of skin wounds, reduced scar tissue, and improved outcomes after injury to heart, kidney and brain tissue. The proposed mechanisms include reduced inflammation, new blood vessel growth, and changes in how cells at the wound edge behave.

Human trials are underway for diabetic foot ulcers, burns and some inflammatory conditions. Results so far are preliminary. None of these uses is approved anywhere as a standard treatment, and any clinic offering exosome injections for injury or disease is operating outside approved practice.

Diagnostics

Evidence: established in principle; a few tests in use

Because exosomes carry molecules from the cell that released them, the exosomes in a blood or urine sample can reveal what is happening in tissues that are hard to biopsy. This is the basis of "liquid biopsy" approaches.

A small number of exosome-based tests are commercially available, notably for prostate cancer risk assessment, and many more are in development for other cancers, neurodegenerative disease and organ rejection. This is the area of exosome science with the clearest path to routine clinical use, and it has nothing to do with skincare.

Drug delivery

Evidence: speculative to early

Researchers are loading exosomes with drugs, RNA therapies or gene-editing tools, on the theory that a natural vesicle will be tolerated better and reach targets, including the brain, that synthetic carriers struggle with. Several engineered-exosome therapies have entered early clinical trials.

The challenges are manufacturing at scale, loading cargo reliably, and proving that the vesicle adds something a lipid nanoparticle could not. This is a decade-scale research area, not something you will find in a product.

On injections and IV treatments

Some clinics offer exosome injections or infusions for anti-ageing, joint pain, long COVID, autism and other conditions. In the United States these are unapproved biologics, and the FDA has issued warning letters and public safety communications after reports of serious infections and adverse events. Products sold for these purposes have not been shown to be safe or effective. If you are considering one, ask whether it is being given under an FDA-cleared clinical trial and, if not, treat that as a serious warning sign.
